Our panel of doctors conducts medical screening tests and examinations required for students, tourists and permanent resident/ immigration visas.
Besides, the Vaccine Centre under the care of Adventist Lifestyle Centre also provide vaccination services to students, travellers and general public.

Important Information
- Fasting is NOT required for this medical exam
- Kindly inform our staff if you need X-ray FILM for your scholarship application (for example JPA).
- The validity of the certificate is 6 months.
Pregnant women (NOTÂ recommended to take chest X-ray during the first trimester).

- Postpone the chest X-ray (and TB clearance) until after delivery.
- Opt for TB clearance based on negative cultures from three appropriately taken sputum samples for Smears and Culture test Result of Sputum Smears and Culture will take up to 10 weeks.
- Continue with chest X-ray with protective shielding.

- Valid original national passport
- One (1) recent passport size photo (of any background colour). It will be used for your medical certificate.

- Applicant‌ ‌MUST‌ ‌know‌ ‌which‌ ‌Certificate‌ ‌they‌ ‌intend‌ ‌to‌ ‌apply.‌ ‌Kindly‌ ‌refer‌ ‌to‌ ‌Immigration‌ ‌New‌ ‌Zealand‌ ‌if‌ ‌you‌ ‌are‌ ‌not‌ ‌sure‌ ‌of‌ ‌the‌ ‌type‌ ‌of‌ ‌Certificate‌ ‌required‌ ‌for‌ ‌your‌ ‌application‌.
- The‌ ‌health‌ ‌examinations‌ ‌that‌ ‌appear‌ ‌in‌ ‌e-Medical‌ ‌will‌ ‌depend‌ ‌on‌ ‌the‌ ‌Certificate‌ ‌selected‌.
- An‌ ‌appointment‌ ‌is‌ ‌required‌ ‌prior‌ ‌to‌ ‌coming‌ ‌in‌ ‌for‌ ‌the‌ ‌medical‌ ‌examination.‌ ‌The‌ ‌screening‌ ‌is‌ ‌only‌ ‌available‌ ‌on‌ ‌Monday‌ ~‌ ‌Thursday‌ ‌at‌ ‌2:00pm ~ 5:00pm.‌
- Applicant‌s aged ‌17‌ ‌years‌ ‌old‌ ‌and below must‌ ‌be‌ ‌accompanied‌ ‌by‌ ‌parent‌ ‌or‌ ‌legal‌ ‌guardian‌.
- If‌ ‌you‌ ‌are‌ ‌short‌-sighted,‌ ‌kindly‌ ‌along‌ ‌bring‌ ‌spectacles‌ ‌or‌ ‌contact‌ ‌lenses.‌
- ‌Fasting‌ ‌is‌ ‌NOT‌ ‌required‌ ‌for‌ ‌this‌ ‌medical‌ ‌examination‌.

Important Information
- Up Front Medical(UFM) examination can be performed before a visa application been submitted to CIC (Citizenship and Immigration Canada) except for applicants under the Family class Excessive Demand Exempt (EDE) Immigration category. This includes spousal sponsorship program (spouse, conjugal partners and dependents) only.
- Applicants aged 15 years old and below must be accompanied by parent or legal guardian.
- Bring spectacles or contact lenses, if applicable
- Fasting is NOT required for this medical examination.
- Female applicants are advised NOT to schedule the medical examination during menstrual period.
- An appointment is required prior to coming in for the medical examination. The screening is only available on Monday ~ Thursday at 2:00pm ~ 5:00pm.
- Drink plenty of water on the medical check-up day (for urine test)

- Examination Referral Letter which contains your IME/ UMI/UCI number or the Health Examination List
- Valid Original Passport with 2 photocopies of passport
- A list of any medication you are taking (including dosage)

- Before coming for your medical examination, you must lodge your visa application at border.gov.au .
- An appointment is required prior to coming in for the medical examination. The screening is only available on Monday ~ Thursday at 2:00 ~ 5:00pm.
- Applicants aged 15 years old and below must be accompanied by parent or legal guardian.
- If you are short-sighted, kindly along bring spectacles or contact lenses.
- Fasting is NOT required for this medical examination
- Female applicants are advised NOT to schedule the medical examination during menstrual period. Otherwise, they will be required to come back one week after the completion of menstruation
- Drink plenty of water on the medical check-up day (for urine test)
